Twin Stim Plus DS5402 Manual: Four-Channel Setup, Programs and Troubleshooting

The Roscoe Medical Twin Stim Plus DS5402 is a portable four-channel electrotherapy device combining the following:

  • TENS
  • EMS/NMES
  • Interferential current, shown as IF
  • Russian stimulation, shown as RUSS

Each output channel has its own intensity controls and accepts one complete electrode pair. The unit can therefore operate with up to eight electrodes. Its central Mode and Set controls select the treatment program and parameters, while two separate timers control the CH1–CH2 and CH3–CH4 channel groups.

This operating guide explains how to:

  • Install the batteries or use the AC adapter
  • Connect one to four electrode pairs
  • Select TENS, EMS, IF, or Russian stimulation
  • Program pulse rate, pulse width, work-rest timing, and sweep settings
  • Understand the two grouped treatment timers
  • Create one or two four-electrode interferential fields
  • Review the patient-compliance records
  • Troubleshoot weak, intermittent, or uncomfortable output
  • Clean and store the system safely

Powering the DS5402

Battery operation

The device uses four AA batteries.

  1. Turn the DS5402 off.
  2. Slide open the rear battery cover.
  3. Insert four batteries, matching every positive and negative marking.
  4. Do not mix old and new batteries.
  5. Do not mix battery types.
  6. Close the battery cover securely.
  7. Replace all four batteries when the low-battery symbol appears or output becomes generally weak.

Remove the batteries if the device will be stored for a prolonged period. The manual warns that depleted batteries may leak and damage the unit.

AC-adapter operation

Use only the specified adapter with a 9 V DC, 800 mA output. Do not connect the lead-wire plugs to any power socket, and do not substitute an unverified adapter merely because its connector appears to fit.

How to connect the electrodes and lead wires

One active channel

One channel requires the following:

  • One lead wire
  • Two electrodes
  • One channel-intensity control

Two active channels

Two channels require the following:

  • Two lead wires
  • Four electrodes
  • Two independently adjusted channel intensities

Four active channels

Four channels require the following:

  • Four lead wires
  • Eight electrodes
  • Four independently adjusted intensity levels

Connection procedure

  1. Turn the device completely off.
  2. Connect both lead-wire pins to the two electrodes assigned to that channel.
  3. Push the pins in fully so no bare metal remains visible.
  4. Insert the insulated lead-wire plug into the correct top socket.
  5. Repeat for each channel that will be used.
  6. Leave unused channels disconnected.
  7. Apply the electrodes before turning the unit on.

The patient-side connector is a standard 0.08-inch female electrode connection and requires legally marketed electrodes appropriate for electrical stimulation.

Skin preparation and electrode application

Prepare the skin

Apply electrodes only to clean, dry, intact skin.

Remove:

  • Lotion
  • Massage oil
  • Perspiration
  • Dirt
  • Adhesive residue
  • Topical analgesic gel
  • Other products that could affect adhesion

Allow the skin to dry before applying the pads.

Inspect every electrode

Do not use an electrode that is

  • Dry
  • Torn
  • Scratched
  • Contaminated
  • Permanently curled
  • Loose at its pigtail
  • Lifting around the edges
  • Unable to maintain full surface contact

Apply the pads

Press the entire conductive surface evenly against the skin. Keep the pads separated and use the exact placement supplied by the clinician.

A minimum recommended self-adhering electrode size of 4 × 4 cm. The included pads, which are 50 × 50 mm, exceed the stated minimum size. Smaller electrodes concentrate current over less surface area and may feel sharper or increase skin irritation risk.

Basic operating sequence

1. Connect all required pads and lead wires

Confirm that every active channel has two electrodes.

2. Turn the device on

Press the Power button.

3. Select the modality and program

Press Mode repeatedly to cycle through:

  • TENS B
  • TENS N
  • TENS M
  • TENS SD1
  • TENS SD2
  • EMS C
  • EMS S
  • EMS A
  • IF N
  • IF M
  • IF M1
  • RUSS N
  • RUSS M
  • RUSS M1

4. Enter the prescribed parameters

Press Set until the parameter to be changed flashes, then use the central ▲ or ▼ control.

5. Set both timers

Confirm the left timer for CH1–CH2 and the right timer for CH3–CH4.

6. Increase intensity one channel at a time

Use the arrow buttons directly beneath the corresponding channel.

7. Monitor the response

Confirm the intended sensory or motor response and watch for poor pad contact, unwanted contraction, pain, or skin irritation.

8. End treatment safely

Reduce all active channel intensities before turning the device off. Turn the device off before moving or removing any electrode.

How the two timers work

The DS5402 has two treatment timers:

TimerChannels controlled
Left timerCH1 and CH2
Right timerCH3 and CH4

Each timer can be set for 1 to 60 minutes or to continuous.

  • From 1–15 minutes, the timer changes in one-minute steps.
  • Above 15 minutes, the timer changes in five-minute increments.
  • Pressing ▲ beyond 60 minutes will select Continuous mode.
  • Each timer counts down automatically when a timed treatment is operating.

This allows one channel pair to stop before the other. It does not let CH1 and CH2 use different times from each other, nor does it let CH3 and CH4 use different times from each other.

TENS programs

TENS is generally intended to create sensory stimulation rather than repeated muscle contractions. The exact mode, pulse rate, width, intensity, and duration should come from the treatment plan.

ProgramDisplayHow it operates
BurstBGroups nine 100 Hz pulses into adjustable bursts of 0.5–5 bursts per second
NormalNContinuous stimulation using the selected rate and width
ModulationMAutomatically changes pulse rate and pulse width through a repeating cycle
Strength-Duration 1SD1Modulates intensity and pulse width through a 40% range over ten seconds
Strength-Duration 2SD2Modulates intensity and pulse width through a 70% range over ten seconds

The manual permits a pulse-width range of 50–300 µs and a pulse-rate range of 2–150 Hz in applicable TENS programs. Burst mode uses a fixed 100 Hz pulse frequency while allowing the slower burst rate to be adjusted.

How to program TENS

  1. Press Mode until TENS and the prescribed program appear.
  2. Press Set to display width.
  3. Use ▲ or ▼ to select the prescribed pulse width.
  4. Press Set again to display Rate.
  5. Use ▲ or ▼ to select the prescribed rate.
  6. In Burst mode, set the prescribed burst rate.
  7. Continue to the two timer fields.
  8. Set the CH1–CH2 and CH3–CH4 treatment times.
  9. Increase each required channel intensity gradually.

Changing pulse width, rate, or mode can change how strong the output feels. Reduce intensity before making major program changes.

EMS/NMES programs

EMS is intended to produce a clinician-directed motor response. The goal may range from a visible twitch to a stronger contraction depending on the treatment objective.

ProgramDisplayChannel behavior
ConstantCContinuous output using the selected width and rate
SynchronousSThe active channels contract at the same time
AlternateACH2 follows CH1; paired output alternates rather than occurring simultaneously

Constant mode

Constant mode does not use programmed contraction and relaxation cycles. Adjustable values include:

  • Pulse width
  • Pulse rate
  • Timer
  • Individual channel intensities

Synchronous mode

In synchronous mode, active channels contract together.

The ON time includes the following:

  • Ramp up
  • Sustained output
  • Ramp down

The ON time must therefore be at least as long as the combined ramp-up and ramp-down time.

For example, a five-second ramp requires an ON time of at least ten seconds.

Alternate mode

In alternate mode, CH2 begins after the first CH1 contraction is complete.

The manual requires:

  • ON time at least equal to ramp up plus ramp down
  • OFF time equal to or greater than ON time

These are device-operating requirements, not recommendations for a particular diagnosis.

How to program EMS Synchronous or Alternate

  1. Press Mode until EMS S or EMS A appears.
  2. Press Set to display Ramp.
  3. Select 1–8 seconds.
  4. Press Set to display ON.
  5. Select 2–90 seconds.
  6. Confirm that the ON time is at least twice the selected ramp.
  7. Press Set to display the OFF time.
  8. Select the prescribed rest period.
  9. In Alternate mode, confirm that OFF time is at least equal to ON time.
  10. Set pulse width.
  11. Set pulse rate.
  12. Set the two timers.
  13. Increase each channel intensity gradually.

Do not shorten the rest period merely to produce more contractions. Electrically activated muscles can fatigue quickly, and an inadequate rest interval may reduce later contraction quality.

Interferential-current operation

The DS5402 creates traditional four-electrode interferential fields by pairing two channels with slightly different medium-frequency outputs.

IF treatment fieldChannelsNumber of electrodes
Field 1CH1 and CH2Four
Field 2CH3 and CH4Four

In each pair:

  • CH1 or CH3 remains at 4,000 Hz.
  • CH2 or CH4 operates at 4,000 Hz plus the selected difference frequency.
  • The intersecting currents create the selected beat or interference frequency within the treatment field.

This means that one IF field cannot be created using only one DS5402 channel. Two channels and four electrodes are required. The unit can create a second four-electrode IF field using the remaining channel pair.

IF programs

ProgramDifference-frequency behavior
NormalConstant selected interference frequency from 1–150 Hz
ModulationSweeps within the lower 1–80 Hz range
Modulation 1Sweeps within the higher 80–150 Hz range

How to program IF

  1. Connect four electrodes to CH1 and CH2 for one IF field.
  2. Connect another four electrodes to CH3 and CH4 only when a second field is prescribed.
  3. Press Mode until IF N, IF M, or IF M1 appears.
  4. Press Set.
  5. Select the prescribed interference frequency.
  6. Set the left and right treatment timers.
  7. Increase each active channel intensity gradually.
  8. Confirm that every pad remains fully attached.

Do not arrange the pads so the current pathway crosses the chest, head, or anterior neck.

Russian-stimulation programs

All DS5402 Russian modes use a fixed 2,500 Hz carrier divided into adjustable 20–80 Hz burst packets.

ProgramOperation
NormalConstant selected burst frequency with a fixed 50% duty cycle
ModulationThe duty cycle gradually changes from approximately 10% to 50% and back
Modulation 1Burst frequency moves between 20 Hz and the selected higher value

Russian stimulation is intended to create motor-level contraction. It should not be used as though it were simply another comfortable TENS pattern. Electrode placement, intensity, contraction quality, joint position, and fatigue all require monitoring.

How to program Russian stimulation

  1. Press Mode until RUSS N, RUSS M, or RUSS M1 appears.
  2. Press Set.
  3. Select the prescribed burst frequency from 20–80 Hz.
  4. Set the left and right timers.
  5. Increase intensity one channel at a time.
  6. Observe the muscle contraction and resulting joint movement.
  7. Stop when the contraction is painful, uncontrolled, or no longer produces the intended response.

Using one, two, three, or four channels

Active channelsPads requiredCommon operational use
One2One localized TENS or EMS electrode pair
Two4Bilateral TENS/EMS or one four-electrode IF field
Three6Three TENS/EMS paths using the same selected program
Four8Broad TENS/EMS coverage, two bilateral muscle groups, or two IF fields

A three-channel setup is not appropriate for IF because interferential treatment operates in complete channel pairs.

Intensity should be adjusted according to the response under each electrode path. The four numeric intensity values do not have to match.

Patient-compliance meter

The DS5402 stores:

  • Up to 60 individual operation records
  • Up to 999 hours of accumulated treatment time

View individual records

  1. Turn the device off.
  2. Hold Mode while turning the power on.
  3. Use the central ▲ or ▼ buttons to move through the records.
  4. Review the displayed record number and operation time.

Delete one record

While the selected record is displayed, press and hold Set for approximately three seconds.

View accumulated treatment time

Press "Mode" from the individual-record screen.

Delete all accumulated records

The manual instructs the user to press Set and Mode together for approximately three seconds. The device beeps after deletion.

Deletion is permanent. Record any clinically required treatment history before clearing the meter.

Why is stimulation intermittent?

Intermittent output may be normal or may indicate a connection problem.

It may be normal when

  • TENS Burst is selected
  • TENS Modulation, SD1, or SD2 is selected
  • EMS Synchronous or Alternate is cycling
  • Russian Modulation is selected
  • The treatment timer for that channel group has ended

It may indicate a problem when:

  • A pad is lifting
  • A lead pin is loose
  • The device-side connector is not fully seated
  • A lead wire is internally damaged
  • The electrodes are dry
  • The batteries are weak
  • Output changes when the cord is moved

The manual suggests lowering intensity, checking the connection, and rotating the device-side lead plug approximately 90 degrees. When the output changes with plug movement, replace the lead wire.

The manual identifies worn electrodes, damaged leads, inadequate electrode area, poor placement, detached pads, disconnected cables, and depleted batteries as common causes of ineffective or uncomfortable stimulation.

Why does one channel feel stronger?

Each channel travels through a different skin-and-tissue path.

Differences may result from:

  • Pad age
  • Adhesion quality
  • Skin resistance
  • Electrode spacing
  • Muscle size
  • Motor-point proximity
  • Soft-tissue depth
  • Hair or skin preparation
  • Lead-wire condition

Do not match channels solely by their displayed number. Adjust each channel according to the intended sensory response or muscle contraction.

Why is stimulation sharp or burning?

Turn the device off and verify for the following:

  • A lifted electrode corner
  • Dry or contaminated hydrogel
  • A pad smaller than the recommended area
  • Pads that are touching
  • Excessive intensity
  • An electrode placed over broken or insensitive skin
  • A damaged lead wire
  • A loose patient connector
  • Topical gel, lotion, or oil beneath the pad

Do not continue treatment through a stabbing, burning, or painful sensation. Skin irritation and electrode burns are recognized potential adverse reactions.

Replacement electrodes

The DS5402 includes eight 50 × 50 mm reusable self-adhering electrodes. Useful replacement options include:

Replacement pads must accept the lead-wire pins, provide adequate conductive area, and be appropriate for the selected modality.

Replace a pad when it:

  • Does not adhere across the entire surface
  • Lifts repeatedly
  • Has dry or contaminated gel
  • Produces persistent stinging
  • Has a scratched conductive surface
  • Has a damaged pigtail
  • Disconnects intermittently
  • Has been used by another patient

Reusable electrodes are single-patient accessories.

Electrode care

After treatment:

  1. Reduce all active channels to zero.
  2. Turn the DS5402 off.
  3. Hold the electrode backing rather than pulling its wire.
  4. Remove the pad gently.
  5. Return it to its protective liner.
  6. Store it in its resealable package.
