D-Core Cervical Support Pillow: Full vs. Midsize & Back/Side Sleep

The Core Products D-Core® Cervical Support Pillow is a firm, fiber-filled sleeping pillow built around a recessed D-shaped center, two different cervical-roll profiles, and raised side-sleeping areas.

Instead of asking you to choose only one neck-support height, the pillow can be rotated so either the smaller or larger cervical roll sits beneath the neck during back sleeping. Move onto either raised outer section for side sleeping.

The D-Core is available in full and midsize, with a single firm support level. That makes the main buying decision relatively straightforward: choose the overall pillow size first, then choose the neck roll each night according to the support profile you prefer.

The feature that gives this pillow its name is the D-shaped recessed center.

During back sleeping, the head rests in that center section while one of the two cervical rolls sits beneath the neck. For side sleeping, the head moves from the recessed center onto one of the taller outer sections.

This is a different design from simply placing a cylindrical neck roll underneath a conventional pillow. The support zones are built into one fiber-filled sleeping surface.

Choosing the correct overall pillow size is the first decision.

Height is only a starting guideline. Neck length, overall body proportions, and mattress firmness can affect which size feels appropriate, so full should not automatically be equated with “adult” and midsize with “small person.”

• You prefer the larger 24 × 16-inch sleeping surface.
• Your proportions or mattress setup make the larger profile a better fit.

• You generally fall within the manufacturer's 4'10"–5'6" starting range.
• You want a smaller overall pillow.
• Your neck length, body proportions, or sleep setup favors the more compact size.

Size determines the overall geometry of the pillow. It is not the same decision as choosing the smaller or larger cervical roll.

The two long edges of the pillow provide different cervical-support profiles.

For back sleeping, it is recommended to start with the smaller-diameter roll. Rotate the pillow 180 degrees if you prefer the more prominent support provided by the larger roll.

Think of this feature as a geometry adjustment, not a firmness selector.

Both are fiber-filled pillows designed for back and side sleeping, but their fitting systems are substantially different.

The D-Core uses polyester fiber, which makes its adjustment strategy fundamentally different from our foam cervical pillows.

The material changes how the pillow behaves.

Neither construction is inherently better.

Choose the D-Core when you specifically want fiber fill plus structured cervical geometry. Choose a foam model when a more fixed contour or interchangeable foam firmness system is the primary goal.

1. Place the pillow so the smaller cervical roll is beneath the neck.
2. Allow the head to rest within the recessed D-shaped center.
3. Use that orientation first while becoming accustomed to the pillow.
4. If you prefer a more prominent support profile, rotate the D-Core so the larger cervical roll is beneath the neck.
5. If necessary, alternate with your previous pillow during the adjustment period.

A cervical support pillow can feel substantially different from a conventional pillow and recommends gradual adjustment rather than immediately forcing prolonged use.

For side sleeping, move the head away from the recessed center and onto one of the raised outer sections.

Those side sections provide a different sleeping height than the central back-sleeping area. This is why the D-Core should not be treated like a flat pillow where the same surface is used regardless of sleeping position.

The D-Core is filled with 100% virgin polyester fiber inside a cotton/polyester shell and is listed by Core Products as not made with natural rubber latex. It is assembled in the USA from globally sourced materials.

The pillow fits a standard pillowcase. Current care instructions specify cold hand washing with mild soap followed by flat drying; do not machine wash or machine dry.
