Important replacement note
Starburst electrodes are not EMPI-branded products. They are the closest current premium replacement based on the following:
- Matching sizes and shapes
- Silver-based current distribution
- Aloe hydrogel
- Four electrodes per package
- Reusable construction
- Pigtail-style lead-wire connection
The Starburst line is coming from the same manufacturing facility known for EMPI-style electrode quality. (Balego)
What were EMPI StimCare Premium electrodes?
EMPI StimCare Premium electrodes were reusable self-adhering electrotherapy pads for compatible TENS and NMES systems. Their distinguishing features were conductive silver ink that spread electrical current across the electrode surface, thick aloe-based gel, repeat-use adhesion, a two-sided storage liner, and construction that EMPI described as latex-free.
| Historical StimCare feature | Purpose |
|---|---|
| Conductive silver ink | Helped distribute electrical current across the pad |
| Aloe-based gel | Provided adhesive and conductive skin contact |
| Reusable format | Allowed repeated use by the same patient |
| Double-coated storage liner | Permitted storage on either side of the liner |
| Four-pad package | Supported common two-channel electrotherapy setups |
| Multiple shapes | Allowed selection based on body area and coverage needs |
Because electrode hydrogel is consumable, owners of otherwise functional EMPI units eventually need a current-manufactured replacement, even if the stimulator itself continues to work.
Why Starburst is the closest premium replacement
Starburst Electrodes are designed for compatible TENS, NMES, EMS, and FES equipment and are sold in the same four main sizes formerly offered in the StimCare Premium family.

Will replacement pads connect to an EMPI device?
Electrode compatibility has two separate parts:
- The lead wire must fit the EMPI device.
- The electrode end of that lead wire must fit the replacement pad.
Many legacy EMPI safety lead wires have an EMPI-specific device connector at one end and standard 2 mm pin-style electrode plugs at the other. In that setup, a current pigtail electrode designed to accept 2 mm pins can often be used without replacing the EMPI lead wire.
Compatibility checklist
| Check | What to confirm |
|---|---|
| Device-side connector | The existing lead wire still fits the EMPI unit |
| Electrode-side connector | The lead wire ends in two approximately 2 mm pins |
| Replacement pad | The pad has compatible pigtail sockets |
| Pad size | Similar to the size previously prescribed or used |
| Lead condition | No cracks, loose pins, exposed wire or intermittent output |
| Device condition | The unit operates properly and has not been recalled or damaged |
| Treatment guidance | Placement and intensity remain appropriate |
Do not purchase a pad based only on the EMPI brand name. Compare the physical connector with the replacement electrode or consult Balego customer service.
Our TENS Electrode Buying and Compatibility Guide provides additional guidance on matching pigtail pads, 2 mm pins, lead wires, sizes, and device connections.
What if the old EMPI lead wires also need replacement?
A new electrode will not correct a cracked, loose, or intermittent lead wire.
We currently carry selected EMPI safety replacement lead wires for compatible legacy devices. Device-side EMPI connectors vary, so confirm the stimulator model and original lead-wire number before ordering.
Do not substitute the general 45-inch TENS/NMES lead wires unless the 2.35 mm device-side plug matches the unit. Their electrode ends use standard 2 mm pins, but their device connection is not universally compatible with EMPI equipment.
How to change from StimCare to a replacement electrode
When switching electrode products:
- Turn the stimulator off and reduce both channels to zero.
- Match the old pad’s size and shape as closely as practical.
- Confirm the replacement accepts the lead wire’s pin connector.
- Apply it to clean, dry, intact skin.
- Begin again at zero intensity.
- Increase the output gradually.
- Do not assume the old intensity setting will feel identical.
A replacement pad can feel different because hydrogel thickness, conductor design, surface area, adhesion, and current distribution vary between electrode lines.
Electrode size matters
At the same device setting, a smaller electrode generally concentrates stimulation over less skin area, while a larger electrode spreads it across more surface area. Balego’s electrode guide therefore recommends matching the original dimensions whenever possible and readjusting intensity after any size change.
Electrode use and care
Current replacement pads should be used according to their packaging and the electrotherapy-device instructions.
- Apply only to clean, dry, intact skin.
- Remove lotion, oil, sweat, and topical medication.
- Handle the electrode by its backing or edge.
- Do not pull it from the skin by the pigtail wire.
- Return it to its protective liner after treatment.
- Store it inside the resealable pouch.
- Assign reusable pads to one person.
- Replace a pad when it dries, curls, becomes contaminated, loses adhesion, or produces uneven stimulation.
- Do not add conductive gel to a pre-gelled self-adhering electrode unless its instructions specifically permit it.
- Do not cut an electrode into a different size.

No. The products are discontinued, and they will not be restocked.
What is the best EMPI StimCare replacement?
Starburst electrodes are the closest premium replacement for the four original StimCare sizes because they combine matching dimensions, silver current distribution, aloe hydrogel, reusable construction, and pigtail connections.
Are Starburst electrodes genuine EMPI electrodes?
No. They are current Uni-Patch Starburst products rather than discontinued EMPI-branded inventory. They are the closest premium EMPI-style replacement.
What replaces EMPI 199657-001?
Choose the Starburst 2" Round, Model 627SB / EP85340.
What replaces EMPI 199658-001?
Choose the Starburst 2" Square, Model 616SB / EP85345.
What replaces EMPI 199659-001?
What replaces EMPI 199660-001?
Choose the Starburst 2.75" Round, Model 628SB / EP85350.
What replaces the EMPI 1.375" round electrode?
There is no exact Starburst equivalent. The closest compact current Balego option is the 1.25" round Balego electrode. Because it is slightly smaller, confirm that the different surface area is acceptable.
What replaces the 4" × 7" EMPI low-back electrode?
Balego does not offer an exact replacement with the same dimensions and integrated 32-inch lead. Compare the Advantrode Elite 4" × 6" Butterfly and Backlasp Electrodes, but verify the changed shape and connection before ordering.
Can I use Starburst pads with an old EMPI TENS unit?
Often, yes, when the existing EMPI lead wire ends in standard 2 mm electrode pins. Confirm the physical connection before purchase.
Must I replace my EMPI lead wires?
Not necessarily. Replace them if they are cracked, loose, intermittent, corroded, missing, or incompatible with the new pads.
Are all EMPI devices compatible with the same lead wires?
No. EMPI used several proprietary device-side connector systems. Match the device model or original lead-wire part number.
Can I continue using very old unopened EMPI pads?
Electrode hydrogel can age, dry, or lose adhesion during storage. A sealed package does not guarantee that discontinued old stock will provide satisfactory contact. Current-manufacture replacements are generally the more dependable choice.
Why can new pads feel stronger?
A new electrode may provide more complete contact and lower resistance than an old, dry pad. Begin at zero and readjust the intensity gradually.
Can I add electrode gel to a replacement pad?
Normally, no. Starburst, Classic, R Series, Balego, and 696SS pads already contain self-adhering hydrogel.
Are these pads reusable?
The recommended replacements are reusable by the same person while the gel remains clean, intact, comfortable, and adequately adhesive.
Can reusable electrodes be shared?
No. Assign each set to one user.
